How to unlock the Lights Out achievement in Kingdom Come: Deliverance II - Definitive Guide
In order to unlock the ‘Lights Out’ achievement, you basically need to take down an enemy that’s looking for you - i.e. the white rabbit icon at the top of the screen. You can do this by throwing stones (crouched and hitting ) or by making a noise that the guard has to investigate. When he does, position yourself behind him and hold the

The perfect quest to do this in is the last quest in Trosky, “Storm,” as you’ll have to escape through the Apollonia woods. This is perfect because a.) it’s dark; and b.) there are loads of enemies patrolling the area on their lonesome. So, throw a stone, make some noise, and then slip behind him to slit his throat.
Once you do that, the ‘Lights Out’ achievement will unlock.

What I did was find an ememy at an enemy camp (I used the camp on the other side of the lake from the nomad camp) I unequipped all my armour and drank a save potion.
I then snuck behind a guard pressed Y to whilstle whilst instantly holding L trigger to initiate the kill. It took a few goes but the whistle works to get them into a search state. I couldn't get it to work on the guard that sits down and used one patrolling. My sneak wasn't too high so should be able to do this early game.
