Super easy achievement. It literally takes 5 min to get it.
Once you get access to the Trosky Castle (From whom the bell tolls main quest done), go speak with Father Nicodemus (the priest of the castle). He's usually found in the chapel, in the highest tower of the castle, near the room where you found the captain in the aforementioned main quest.
Once you found him, ask him to go on a Pilgrimage to get rid of your sin. He will tell you to go a shrine on the road. Just go there (a quest named Pilgrimage will appear), pray and at the end of the prayer the achievement will pop!
Literally takes 5 mins.

Webb If you get caught after causing a serious crime in Kingdom Come Deliverance 2 you’ll have the option of a few pretty severe punishments. Say, like us, you accidentally murdered two villagers who turned on you and you just defended yourself. When we stepped foot in the region again and got spotted by guards, they gave us a few options.
Mainly, you can fight and make things worse, you can head to the pillory for a few days, get caned, pay a steep fine (it was well over 2,000 Groschen, which early on, we did not have) or finally, you can get branded.
Naturally we chose to get branded - which takes 48 hours to heal and for those 48 hours you’ll get treated like the scum of the earth… that, and Henry is branded forever, which is kind of extreme.
After being branded, we headed to Trosky Castle and spoke to Father Nicodemus, who said we can seek salvation by seeking out a shrine to pay penitents and atone for our sins. This starts the task, “Pilgrimage.” You can probably get this task without getting branded, to be honest, just to improve your status in the world if you’ve ever been a bit of an annoyance.
Then, head to the shrine (where the waypoint is on the map) for the “Pilgrimage” task, where you can pray. If you see the prayer through to its conclusion (don’t try to skip it), the ‘Penitent One’ achievement will unlock. Better than paying thousands in fines, right? Right!
