How to unlock the Infernal Justice achievement in Kingdom Come: Deliverance - Definitive Guide

During the mission: The Madonna of sasau
You start the quest line by visiting Johanka by the monestary, and ask:
- Peculiar dreams?
- Have you told anyone about it?
- I think youre right
- Alright, I'll do it

Now fast travel to skalitz, and follow the objective to the west until you find a mine entrance.
Inside the mine, interact with everything you can.
Outside above the mine entrance you will find a hole in the ground. Interact with the blood.
A bit further up the hill, you will find a skalitz guard. Talk to him. (nomatter what you select in the dialogue it ends the same way).
The guard has a key on him, loot it and take the helmet from the chest inside the hut.

Go back to the guard, pick up his body, and carry it to the mine shaft. Throw the body down the mine shaft.

While carrying the body, I fell down the hole and died. Achievement still popped.

Story related but can be MISSED.

This takes place during the quest "The Modonna of Sasau". You'll end up going to Skalitz and looking through a mine shaft to check out Johanka's dreams. Interact with what you find at the end of the mine shaft, then head out and look for the hole in the ground where the bodies were dropped in. Now go and find the person responsible for it.

No matter what your dialogue choices are, it'll end up in a fight where you have to kill him. Once you do, take his key and loot the chest in the hut. Then grab his body and go back to the hole in the ground and drop his body in it.
