Is this cumulative (i.e., is this required to be done in one play through)?

I think they all need to be done during one play through (I tried to get the twenty stealth kills by reloading the same area, but after the first time the percentage on my achievement tracker never changed).

You'll be walking a lot until you get a horse. If you don't have it by then, keep walking.

You should earn this naturally throughout the game. If you want to get this out of the way, don't use a horse and walk everywhere until you get it unless you're on a timed quest. Using fast travel does not count.
