KINGDOM HEARTS HD 2.8 Final Chapter Prologue

The Code Break reality shift can only be activated in The Grid. In this reality shift you are able to hack into enemies and machinery to cause a number of effects. You need to push the X,Y,A or B buttons as they show on the screen to cause the related effect.

This is simple enough to achieve but will take some time to have all of the available commands pop up. Keep activating reality shifts until you have done at least one of each. Below is a list of all commands required and where they can be found:

Auto Destruct – Turrets.
Moving Spark – Enemies.
Prize Box – Random on enemies.
Prize Shot – Random on turrets.
Switch Target – Enemies or Turrets.
Transport – Transports in Throughput or Rectifier 2F.
Unlock – Access panel in Docks (Upper).

Once you get to The Grid, you gain a new reality which. When a Dream Eater is beaten down enough, you'll get the reality shift target. Press and together to start it. Now press the random buttons it tells you. You want at least one from each color. If you do it enough times, eventually you'll get the achievement.
