This requires you to beat all five islands in a row without losing/restarting! The islands don't have to be defeated in order (I beat island 1, 5, 4, 2 & 3) so make sure you keep track of which islands you've beaten during the streak as the game doesn't visually tell you.
The achievement didn't unlock for me until after I had beaten all five islands and then started a sixth game.
