How to unlock the Foiled Again! achievement in Kingdoms of Amalur: Re-Reckoning - Definitive Guide
The achievement is unlocked on the 100th successful parry. You must have a shield equipped. A parry is when your block is timed to coincide with the enemy's attack. Enemies have animations preceding their attacks. So, it's a matter of learning their 'tells' and blocking at the same time as the attack. If your timing is not precise, you merely block. When it is precise, you'll get audio and visual feedback signifying a successful parry. If you are a melee fighter, you will parry often and you will unlock the achievement through normal play.
If you are a ranged or mid-ranged fighter--staff, scepter, bow, chakrams, etc.--you are normally out of parrying range during normal gameplay. The Boggarts are good enemies to grind this achievement out. They have two attacks--a jumping attack and a side by side attack. Both attacks can be parried. The animation that precedes either type of attack is very distinctive and clearly telegraphs the strike. It is easiest against one opponent but you'll reach the 100 count faster with two or more. A successful parry knocks back all enemies in your vicinity so you do not assume unacceptable risk fighting more than one opponent.

Note: If you are using an Elite controller or other controller with a stop that shortens the pull, this may impede your blocking and parrying abilities. Make sure the stop is not used and that the has the full range of motion.
Just as an enemy attack is going to land, press to block, though you must have a shield equipped. If you parry successfully, time will slow down slightly and the enemy will fall back a little. Do this 100 times and the achievement will unlock. If you parry throughout the game, this won't take long. You do don't think about it or don't use a shield, equip one and go back to the first area and parry everything to unlock quicker.
For those, who need this trophy in a very fast manner like me (12 days of Christmas challenge cause of its ratio), you can do this in the very first level. Play on casual difficulty, go ahead til you get your first shield. Equip it using the option button and from then on parry with LT short before the enemies incoming attack. Don’t use your sword anymore, keep parrying with the exception of enemies using bows. This way I managed to get this as my first trophy in the game, should take you 20-30 min!
To parry an attack you will need to have a shield equipped and then press right before an enemy's attack would hit you. You will know this worked properly if you take no damage, the game's speed slows for just a second, and the enemy is knocked back. You can farm a number of parries on a single enemy, though they do take minor damage. Wolves and bandits telegraph their attacks and are very easy to parry.
