How to unlock the Good as New achievement in Kingdoms of Amalur: Re-Reckoning - Definitive Guide
The achievement is unlocked the first time you fully or partially repair a degraded weapon or other piece of equipment. You can use a repair kit or employ the services of a blacksmith. Repair kits are found as loot in chests during the prologue and elsewhere. The first blacksmith you come across is in The Corner Shop in Gorhart. Some merchants sell repair kits. More than likely, though, you will need to repair a weapon before you come across such a merchant. The closest one is Ochdran Fall, the quartermaster, in Shieldring Keep.
Weapons and armor degrade. If you open up your inventory and look at a weapon, a shield, or a piece of armor, you will see an anvil icon at the top left followed by a ratio in the form XX/YY. XX is the current state compared against the pristine item value, YY. When a weapon’s durability falls to 0, it will remain equipped until you remove or replace it. However, it will become ineffective. To restore its strength, repair it using a kit or by employing the services of a blacksmith. In the very short video below, the Twinned Souls chakrams have degraded to 1/20. Using a repair kit, they are repaired to a 10/20 condition unlocking the achievement:

In the first town you come to after the Well of souls and talking to the guards at
. on the right hand side of the entrance to the town is an blacksmith. Nter the building. Behind a desk on your right is a weaponsmith. Talk to them and then press to enter the repair screen. As ling as you took damage a little in the well, you'll be able to purchase your first repairs to a weapon or piece of armour.

I personally wanted until I finished two side quests, one which lead me back to Gorhart, then walked into the shop with the blacksmith and paid for my items to be repaired.

By the time you reach Canneroc (a town in Webwood), you should have found a Repair Kit (assuming you've been exploring ruins), if not they can be bought from Beckry as well (you can also buy them from Ochdran Fall at Shieldring Keep in Yolvan).
Go to "Inventory", then "Items", and scroll down to find your Repair Kits. Select it and it will allow you to pick an item to repair.
The durability of all items not at 100% will appear, pick one and the achievement will unlock.

You will need a Repair Kit for this achievement, which you get right away in the tutorial. As you use weapons to attack or get hit by enemies, your equipment will degrade. Use a repair kit from your inventory and select the degraded equipment to restore it and unlock the achievement.
