How to unlock the Reckoning Rampage achievement in Kingdoms of Amalur: Re-Reckoning - Definitive Guide
The achievement is unlocked when you defeat five human-sized or larger enemies while in reckoning mode and fateshift. Smaller enemies like brownies, sprites, or boggarts do not count towards the achievement. Make sure the purple gauge at the top left of the screen is full. You can check by pressing and looking at your "Fate" percentage in the center of the screen--it must be 100%. Engage a group of five human-sized enemies or larger. Initiate reckoning mode by pressing & simultaneously. An prompt will appear next to each enemy you defeat. Once you have defeated five or more enemies, go up to any one of them and press . You will be given a button prompt that you can press rapidly to get a percentage increase on experience earned. The 'essences' of the defeated enemies will flow into you. Provided you have five or more of the defeated enemies' essence streams, you will unlock the achievement.
The 40 second video below occurs during The Commendation quest--the first quest in the Warsworn faction quest line. The player character goes north of Gorhart to Osgar's camp to retrieve a dagger. As he approaches the camp from the south, Red Legion Bandits attack. He waits until six join the fray and goes into reckoning mode. After defeating all six, and before the purple gauge runs out, he fateshifts the defeated enemies unlocking the achievement.

There is no need to try to get this achievement early. Later in the game, reckoning mode will be used to survive lethal assaults of large groups of enemies. The Legend of Dead Kel questline, for example, has many such opportunities. So, if you don't get this achievement deliberately, it will most likely come naturally during the course of play.

When you have a full Fate meter (the purple meter below your Health, but above Mana), you'll need to find a group of enemies.
A few places early on, the bandit camp in Yolvan has 5 to 7 Bandits that can be group together, otherwise there are spots where you can get 5 Boggerts can be grouped together.
When you have at least 5 enemies, hold both triggers ( + ) and bring all enemies to near 0 Health. You won't be able to kill them, so just keep wailing on all enemies until they are all stunned.
Press A next to an enemy that is stunned (you'll be given a prompt) and it'll give you a button to mash. Do so and you'll unlock the achievement.
I had some enemies die while I was in Reckoning Mode, so they didn't count. Not sure why this happens, it shouldn't, but it does. You just have to try again, sadly.

Travel to Gorguath and just to the right of it (from normal map view), there's a bandit camp. Hit Y to switch to Local Map and scroll around and you will see the bandits. I encountered exactly 5 of them.
Make a save and reload as necessary.
Run around to have them chase you so you can get all five close then enter Reckoning Mode. Attack until each enemy has an A above their head then hit A on one of them and follow the prompts.
If the achievement doesn't unlock reload your save.

Fateshift is unlocked shortly after the tutorial before you reach the first town. As you kill enemies, the purple bar at the top of the screen between your health and mana will fill, and once maxed out you can press + to activate Fateshift. This slows down the enemies and gives you a huge boost to your damage. Enemies "killed" while in Fateshift will have an prompt over them. Use Fateshift with 5+ enemies present and get them all to the point where they can be killed, then press when next to one of the enemies and they will all be Reckoned and the achievement will unlock.
Note: Smaller enemies like brownies will die immediately during Fateshift and can't be reckoned, so use human enemies to be safe.
