This achievement requires that you visit all areas in the game...technically. If you do the story and the in-game DLC packs are they become available, you should unlock this achievement in the Upper Tunnels area as I did, which is part of the "Back to the Source" DLC you can buy from the 4th magazine. However there are a few more areas past the Upper Tunnels. You'll visit every area if you do all the main/side quests in the main game and DLCs.

For this achievement you will need to visit all the regular main story locations as well as the 'DLC' one. The 'DLC' can be purchased with Gold in the Magazine section. There are 3 'DLC' for this game, and they unlock depending on the level of your character.
