To summarise all the solutions and comments I will make an all inclusive one.

If you fall off a cliff or into any water, or get killed by an enemy then you will not get this. If any of these occur then you should reload your save which will stop the game from counting your death.

According to the guide post by MetalGeekMark, you do not need to get the QTEs to get this. If you miss one you can keep playing. I have not tested this myself, but he has had others confirm it.

Obviously playing on the easier difficulties will help, but the game is easy enough that you shouldn't need to. The easiest points to die is falling which is the same on any difficulty

There are some cheats which will make this easier:

Infinite Chi - Down, Right, Left, Up, Down
Invulnerability - Down, Down, Right, Up, Left
4x Damage Multiplier - Up, Down, Up, Right, Left

For clarification, I am sure I just reloaded the save on death and still got this, but it has been over 2 years so can't confirm if it was just a reload or a full quit. Apologies that I can't confirm further.

Grate guide mate but about the cheats.. I never used a cheat once and i been told that some cheats stop cheevos for a few games? Will it still be safe to cheat on this without the whole worry about not getting the achevement?

This game does not disable the achievements with the use of cheats. I played through once without them and then once with them and got all unlocks fine

I did the same as Kornfan2007.. Played first time through and forgot to add the cheats, then played 2nd time through with the cheats on.

I reloaded everytime i died and i didn't get it. Do i have to select new game for it to work, because i clicked on an old save and started from the first level and got to the credits and it didn't pop.

i played the game on dragon warrior without dying and didnt miss any button events but i still didnt get the cheevo.

Just want to make sure I understand if I die! I just go to the load menu and reload the level and it won't count my death???

I only died 2 times, reloaded both times, and still didn't get the achievement. Was I supposed to Quit Game entirely? I doubt that's the reason since doing so requires you to Load the same file all over again. Ughn, this disappointing.

And you wanna know what's the most infuriating part? They make you use a NEW Save File in order to get this, and because of that, the game assumes you haven't seen any of the cutscenes yet, when frankly I'm SICK of seeing them having seen them so many times. They aren't even really interesting. They're just time-consuming. I'd be able to blast way faster through the game without them killing time Before, During, and After every level. -__- I'm only playing this game for the achievements. It's actually good for a cartoon movie game, but man I'm sick of it after squeezing out every one of these achievements except for This one.

Really easy achievement I can confirm that you still get the achievement if you miss the QTS I missed a lot of them

Every time I died or missed a quicktime event, I dashboarded and reloaded the entire game. I fell a few times on the platforming bits and missed like four separate quicktimes, and still got the achievement in the end by dashboarding every time I screwed up.

What if, you fall on a level, go back to the menu screen, and begin from the start of the previous level? Will this "overwrite" your death on the level? I'm going to try this.

I just did reload of save when I fell on a level (I am pretty sure anyway). It has now been 3 and a half years since I did this so my memory of exactly what I did is not fresh

I can also confirm if you die and it puts you back in the game, if you press start and then load from your last checkpoint, you can still get this achievement.

Yeah, I reloaded the save and still got it. Only really died in those lilypad levels... I thought I might have died at the same time it auto-saved once, but luckily I still got the achievement.

when you fail a boss fight quick time event does that count as a death?

I died a few times to falling in the water/off an edge, and even though the death animation still went through, I instantly reloaded and still got the achievement at the end.

You definitely DO NOT need to get the quicktime events right to get this. I failed pretty much every one of them at least once and still got this achievement. All you need to do is use the cheats and reload when you fall off the map.

Double verified. I did this the other night and failed a few of the quicktime events. Achievement Unlocked - Invincible.
