As your finishing your visit to the Hospital a short cut scene will play showing June leaving with her lawyer. The Dectectives decide to follow her to see where she goes.
Things to note:
If the icon starts to fade you're to far away.
If the icon starts flashing you're to close.
Also don't use the siren
Try to avoid crashing into cars
Once you've followed her around for a short time, she'll come to a stop outside a cafe and go inside. Cole will jump out the car. What your want to do before going in is use
to take cover by the door so you can see in. Once the person in front of June has hung up, wait until she turns to use the phone, at this moment press again and walk inside, the second booth on the left has a newspaper on it, press 
to interact with it. Cole will sit down and listen in on the conversation. After she's finished the call wait for her to leave first before getting up from the booth. Your be given your next objective and the achievement.

The first time you tail a suspect is in the case 'The Fallen Idol'. You'll come to a point in the case where June Ballard will get into a vehicle and you'll need to follow her in your car. Make sure you stay at a safe distance, so about a half a block or a little more. If you go any further or you lose sight of the car the red car icon on your mini map will start to flash, which is indicating you've lost sight of your suspect! If this happens, simply catch up a little or turn the corner so you can get a view on the car again.
You'll want to make sure you don't get to close to the suspects vehicle activate your siren or drive like a mad man as the suspect will spot you and you'll fail your objective.

Please note that if you fail once, you will need to start the case over. There is another chance to get this trophy and that is when you are going for The Moose trophy. Please see that trophy for more info.
The very first place that this trophy can be achieved is in the case The Driver's Seat on Traffic Desk. Simply play the case until you reach the interview with Frank Morgan in Cavanagh's Bar. At this point make sure you guess the questions WRONG that way it will force a tail on Frank Morgan. For both questions simply select TRUE as the answer and you will have to tail Frank Morgan to his apartment where Adrian Black is.
The second place that it can be achieved is on the case "The Fallen Idol" on the Traffic Desk. You will tail June Ballard until you reach a cafe. At this point you must get inside the cafe without alerting her. At the entrance of the cafe press 
to go incognito and then move over to the booth with the newspaper on the left of the entrance and sit. At this point the trophy should pop-up.

After you talk with Jessica at the central district hospital, at the exit (after the cut scene), you will be asked to follow June Ballard. Follow her slowly in the car, keeping a large distance. After June enters the cafe, run up to the entrance and stand behind the wall (use cover
). Watch June, as soon as the telephone booth is free, she will start calling it.WAIT!!! until she picks up the phone (before that she will turn around and look back a couple of times) and only after that go into the cafe. Go to the table with the newspaper (second from the left) and sit down
for him.When you sit down the trophy will appear
If for some reason you are noticed, then restarting from the checkpoint will not help to get the trophy (you need to start the case again).
