This is for getting all the achievements. This one popped right after unlocking the final ending for me.
All of the achievements can be earned in a single playthrough if multiple save points are created. You can have up to 15 save slots. I would advise creating a new save regularly in case you miss a collectable. There are miss-able collectables and choices that must be made correctly. There are also 4 different endings.
This video guide is great. It WILL give you an orange prompt on all of the important steps needed for all endings.
Limited saves: no
Difficulty: easy
MISSABLE At the very start, make sure to grab the coin and throw it into the fountain.
Grab the coin from the bottom-left of the main hall and the tooth from the bathroom.
DO NOT GIVE THE GUY IN THE BASEMENT THE ROPE.

Answer: I have not seen her
MISSABLE Grab the coin from one of the child's rooms before getting the crank.
MISSABLE Grab the tooth right after finding the crank
Right after entering the garden, make sure you talk to this guy. Technically this is missable but pretty hard to miss him.
A little while later, you will come upon a lady in a green dress.
Answer: "I am looking for my wife"
Tooth near a big statue in big area.
Axe code is 4213; not needed for any achievement

After waking from the dream, read the note near the coffin and grab the heart
Make sure to use the heart pendent on the painting
