How to unlock the Immortalized in my heart achievement in Layers of Fear (2016) - Definitive Guide
This video will show you the location of all collectibles (Family Mementos, Rodent Drawings and the Memory Recollections). It's not my video, I take no credit, but I found it helpful.
Also, note that collectibles carry over. So each time you start a new game or use chapter select, only the collectibles you haven't grabbed yet will be there for you to find. So if you miss any, just grab it on your next run (you'll need at least 3 if you're going for the "all endings" achievement) or jump to the right spot with Chapter Select.

Oh, it just occurred to me too, in the video, the guy mentions that he somehow has the last memento despite never collecting it. It's a small chance, but maybe you have a memento showing up in the album that you didn't actually pick up? It seems to be the actual act of picking up the collectible that registers for the achievement, so if you have one in the album that you didn't actually pick up, it's not going to count for the achievement. It should still be out there for you to pick up if this the case, so it might be an idea to check the locations and make sure they definitely aren't there. Though note that one of the mementos (the wedding photo with the faces scratched out, which is in the desk of the room where you get locked in and can collect the cane embedded in the wall just outside) always seems to re-appear and can be collected again, even when you already have it.

What does your achievement guide show? Like, what percentage of the achievement is show as done? There's a glitch of sorts on the Xbox One where an achievement is done, but gets stuck at "unlocking...". If it says this for you, you will eventually get the achievement. Sometimes it (seems to) help if you leave the console on for a few hours doing nothing, but either way, you will get the achievement in this situation. You won't get the "achievement unlocked" thing pop up, it will just randomly go from locked to unlocked at some point. If you look and this isn't the case for you, and you don't have 100% on the achievement, then the only thing I could suggest is maybe deleting your save and re-collecting everything in the hope that this does it for you.

the guide above is great and should be your go to guide if you start a fresh game and want to go for all the collectibles.
If you are like me though and you have made your run already just to notice that you screwed up and missed a single memento I can recommend the video below. The advantage of the video below is that it only focuses on the memento rather than on all the collectibles in the game.
To make things even easier, you can check which ones you missed if you restart any of the chapters and open the photo album on the desk to your right (if facing the canvas).
