with the new update 4th august 2012 at the main menu screen go to the mutation button press Y to cycle through the mutations play one of each of the originals ( just load up the map and quit out)
The fastest way i found was to set it up as a local server (mine could not conect to the dedicated) load and quit out here is a list of the ones i used to get the achievment

I played Headshot, Four Swordsmen, Chainsaw Massacre, and Iron Man. So all mutations listed on the main menu should count.

I'm so glad they made this easier to obtain after the update. I got the achievement with mutations not listed. I played Headshot, Four Swordsmen, Chainsaw Massacre, and Iron Man. So all mutations listed on the main menu should count.

The counter for my Mutations is stuck on 4 no matter how many I cycle through. Any suggestions for fixes?

Join a mutation every friday afternoon, as its a new mutation per week - you dont need to play, just join.
If you're stuck at 5/6 (like I was for 2 weeks!!), join a mutation game then play versus for few minutes. This SHOULD unlock the last one for you!

I believe the problem is the current mutations going on, are community made, and are not programmed in to count towards the achievement. I was at 4/6 the week before the community made mutations started coming out, and have not gotten any credit for the last 2 months of mutations that I have played.

For anyone still on the path for Mutant Overlord, mutations are now done biweekly, meaning it takes a minimum of 3 months to finish under the best circumstances. They update around noonish and the date of this post was one of the biweekly updates, so plan accordingly.
As for people 'bugged' in their progress, perhaps realism versus is the issue? I played Versus Survival and Lone Gunman in June, Healing Gnome and Tank in July, none in August because they repeated mutations I had already, then Hunting Party just today. I played Realism Versus last because I knew it was my quick ticket out of the achievement at 5/6 mutations. I was also kicked nearly immediately from the Realism Versus lobby and received Mutant Overlord while staring at the main menu.

For this achievement you need to play 6 mutations, the mutations can be found in the Left 4 Dead 2 menu under their own category. Mutations are currently being updated and changed once every 2 weeks and so at this rate it will take a minimum of 6 weeks for you to unlock the achievement.
However that being said there's a couple of major issues for players working toward this achievement. The first of which is that Valve introduced what are known as Community Mutations do not count toward your mutations count and so in short, if a Community Mutation is put up while you working on your achievement, it won't increase your count and you'll have to wait for the next mutation and hope that it's an "official" mutation. The second piece of bad news is that duplicate mutations do not count twice, for example if you play "The Four Swordsmen (Of The Apocalypse)" and later down the line play it again, it won't count. So in short each mutation can only increase your total count one time.

Now, as stated earlier the game updates the mutation once every 2 weeks. Every Friday the in-game blog will notify you of the mutation change (approximately 15:00 Eastern/Noon Pacific).
There have been reports of people getting stuck at 5/6. It appears the issue is that Realism Versus only counts as .5 (0.5/6). To solve this simply join a game of Realism Versus again and play for a while. During the game the achievement should unlock. This "fix" comes from Ben Legend
To receive credit for playing a mutation you only need to enter the challenge/game. This has been confirmed by multiple people and is useful if you don't have a lot of time in a given week to play.

At the moment, you do not need to wait 6 weeks to get this achievement (perhaps the reason for this was the release of a patch or the developers disabled this option). Mutations can be changed independently, they are open and available from the very beginning. To do this, in the main menu of the game, select the "MUTATIONS" mode, then press To change the mutation, change it to one that you have not played yet and launch any campaign, as well as any chapter in it.
