The best way I've found is to play 1v4 match, with the 1 going for the achievement on the infected team. It can be done with 2 or 3 on the survivor team, but 4 gets it done faster.
Start the versus match on chapter 2 of Crash Course. Let the survivors play all the way to the finale. Mess around, attack them if you want, just make sure you don't kill any of them. Once in the finale area, have them start up the generator and fight the first wave of infected. When you become the tank (this is the reason you are the only one on the infected team, you will become tank every time), have 3 of the survivors stand near one of the cars. Hit the car at them and take them down. Have the 4th survivor pick them all up, then do it once again. When they are revived this second time, they will be in the black and white near-death zone. Let them heal with the health packs nearby (hopefully they havent used them yet) and then knock the the same 3 down twice again.
Keep repeating this until they run out of health packs, then just finish them off with one more car hit. If all 4 survivors enter the truckyard with healthpacks, you should be able to get the achievement in one playthrough. If not, just return to the lobby and do it again.
Also, if you can't get 4 people on the survivor team, kill all the bots before you start the generator to make sure that they don't kill the tank.

This achievment popped up when is started up LFD and connected 2 xbox live!
I have no idea what it was and i never played the campaign for it, I diddnt joined a versus game or what ever!

This achievement glitched for me... in a good way.
I cleared my cache for other reasons and the very next time I started up L4D, this achievement unlocked along with No Smoking Section (both popped on main menu, before pressing start).
I had previously played L4D2 and still had a guest signed in, only have the Crash Course DLC.
I previously only had about 5 or 6 smoker kills and absolutely nothing towards the 20 cars.
Let me know if it does/doesn't work and if you have a guest or not, and what DLC you have :)
EDIT: A lot of people are reporting that the achievement pops regardless of having DLC or not. If you don't plan on buying the DLC and still want to 100% the game, then it seems like you'll have to always play offline and NEVER apply a game update- unless this gets fixed.

That was weird, I last played the game May of 2009 and just recently picked up a GOTY (only includes the survival pack and not either of the DLCs). I left the game rubberbanded for about 5 hours yesterday doing the safe room thing for Zombie Genocidest, then took out the disc and played a MoH Airborne session, then went to bed for the night. This morning, I put in Mercs 2 to test the servers, and after having no luck put in L4D to do more rubberbanding for zombie kills. This achievement unlocked after I hit Start at the menu, and immediately before I selected a storage device.

I've heard that people have also unlocked 20 Car Pile-up even though they don't have Crash Course DLC!

although these achievements are known to be glitched and not unlock for some people, system link is not the only way to achieve this. host a friends only game and invite other people who need the achievements on crash course, once your game is set up tap A repeatedly on the 'start the game' option. (try doing it as fast as you can as that tends to work with me) the game should instead of searching for a dedicated server, start the countdown straight away and host a local game where all the achievements are unlockable. the game will sometimes crash when trying to start this!!! but keep trying as it does work and makes it easier. ask me if you need any further help on this...

This is going along with Jacky Boy, but you can do this with just two people but it will take a little longer.
- Do the A button mash as stated by Jacky
- on the second level of crash course you will always spawn as a tank in the finale
- right at the edge where you fall down into the area with the generator there is a car, so have your buddy and his group start the generator and then run to the corner of the yard by the car and fend off the infected
- after one wave you should spawn as a tank, make sure your buddy lines the bots in front of the car, and then as the tank just run around the crate and hit the car, which should take out all four in one hit
- then just replay the finale, and since you have to get 5000 K for one of the achievements anyway there is no rush here

This is just me offering my two cents on the topic given the fact I recently did this with a friend. While I know this has been kind of buggy for some, I had no issues while playing the On Demand version (Supposedly it's GOTY version minus the DLC?), though I also didn't get the achievement simply for starting the game either.
Naturally, you'll have to be playing in versus mode in order to obtain this achievement. Personally, I started a friends-only game on the second half of the stage. My friend and I signed in second controllers in order to make things a bit easier, though it's not entirely necessary. If you are doing this I suggest you have the one person going for this on infected, while the other 3 players are humans.
What you'll want to do is run through the stage and make your way to the generator with all four survivors. You can ignore your infected controller, as it probably won't spawn as a tank until you use the generator. As soon as you see the area with the generator after jumping off of the truck, look to the right besides the carrier and you'll see a truck against the structure. This will be your primary form of attack.
Once you've walked all of your humans here, start the horde and hold out until the player spawns in as the tank. Line 3 of your players up in front of the car and allow the AI (or another player depending on how many you have) revive them. Proceed to knock the car through the three of them again and you should have 6. Then, heal them up with medkits and you can proceed to smack the car through them for more progress towards your achievement. It may take a couple of runs, but the achievement is not nearly as daunting as it may seem.
Side note: While the car is your best bet in order to earn the achievement, the dumpsters in the nearby area also count for hits despite the achievement specifically saying cars. Whenever I struck foes that were on their last legs, it wouldn't count. That said, you should pretty much assume the hits won't count unless you're downing them with the object. Happy hunting!

Becoming a Tank is a bit random. Some say the worst guy on the team that is also far away from other infected will become one. Anyway, when you become a Tank, make sure to hit the cars or trash boxes with the right trigger to have it fly at the survivors. You can do it on-line in versus or with set-up friendly match with your buddies or you can do it yourself off-line. In friendly match you can have the other survivors keep reviving the downed ones to maximize the amount toward this achievement.
