How to unlock the No-one Left Behind achievement in Left 4 Dead - Definitive Guide
This one requires quite a bit of teamwork. Just make sure you have a defendable position of some kind and work to keep everyone alive. Before the evac arrives, make sure that everyone knows where it will arrive at! When the evac finally arrives, pipe bombs are ideal for drawing the horde away so that the survivors can proceed unmolested to the evac vehicle.
Everyone (alive) must get on the vehicle for it to leave and the campaign to end. The first person into the vehicle should lay down suppressing fire for the rest and so on.

This is a fairly easy achievement on either normal, or easy difficulty. Of course doing this on easy difficulty would be the easiest way to do this.
I would suggest getting 3 friends together as AI's are not as reliable as human players, and you can all watch each others backs and stay in the same spot. AI's have a habit of wondering around in dangerous places you don't want them to be.
Also I would suggest doing a level that your team is comfortable with, No Mercy seems to be a favorite among most players, and is arguably the easiest level.
As a general rule you should save at LEAST 1 pipe bomb for when the evac vehicle gets there in order to keep the horde away while your team makes their way there. 2 pipe bombs would be preferable.
There are good defense places for every level, and I usually ALWAYS have all 4 players make it alive IF they listen to where I tell them to go.
Here are the places I like to defend at, and some tips when defending there.
No Mercy:
Have all 4 players wait in the stairwell, 2 upstairs and 2 downstairs.
The people downstairs will have to watch the window on the right as a boomer can tend to sneak up and puke on them.
The people upstairs will have to watch the building with the satellite on it and the left side as smokers have a tendency to go to those two spots.
Hunters will go to either spot, it usually doesn't make a difference and they go either upstairs or downstairs.
When the tank comes have 2 people go to the top next to the mounted gun. Have one person stand on either side of the ladder as far as they can over the edge. If the tank comes from the left it will try climbing the ladder and get stuck making it an easy kill. If it comes from the right just kill it as you normally do.

Once you kill the second tank start running to the helicopter. Throw you're pipe bomb if there are alot of zombies, or someone got boomed. If anyone is at yellow or red health and anyone has pills have the one in yellow or red health take those pills.
Death Toll:
When you get into the house with the radio you will notice there is a door in the back, and 2 more doors, one on either side.
Have 2 people wait in each of those rooms, one's a bathroom and the other is a closet (I think)
There shouldn't really be any problem here except for boomers as they tend to walk in with the horde and someone kills them without seeing them and they are exploded on.
There isn't really any trick to killing the tank here, just take one of the many molotov's, light him on fire and fill him full of lead.
Once you kill the second tank starting running to the boat. Throw a pipe bomb if there are alot of zombies, or if someone gets boomed. If anyone has a molotov, or you're lucky enough to get a gas can at the end of the dock light the end of the dock on fire.

When you get to the gas truck find the wall at the very back and you will find a hero closet, this is a far walk to the evac vehicle, but it is the best place to wait so just make sure to have 1 or 2 pipe bombs.
Have all 4 people wait in the hero closet in the back, 2 standing and 2 crouching. In this level it would beneficial for one person to have the sniper, but it isn't necessary.
Hunters and boomers shouldn't be any problem as you are all close enough to hit the hunter off, and you should see the boomer before he gets to you. Watch for the smoker on the wing of the plane, and he usually will come from the center in a horde of zombies. Look for the green spores.
When the tank comes have everyone get out of the room. Go to the right side of the wall. The tank MAY come from that side, but he usually will come from the left side of the plane, or will come through the center, sometimes walking through the fire.
RIGHT after killing the second tank head towards the evac vehicle, make sure everyone follows the same path as all 3 specials will have spawned here. The boomer is usually on the opposite side of the gas tanker, the hunter and smoker usually are on the same side of the gas tanker as the radio is.
If someone does get boomed, or there is a horde of zombies throw the pipe bomb you've been saving and walk onto the plane. If anyone is at yellow or red health and anyone has pills have the one in yellow or red health take those pills.
Blood Harvest:
When you get to the house with all the supplies if you look to your left you will see a barn. This is where you're going to wait.
Because it's to risky to go back to the house for the medkits use ALL of the medkits on your players with the lowest health.
Have three people go to the barn, and then the last one will alert the rescue vehicle and make his way to the barn. Don't worry, there shouldn't be any zombies blocking your path if you leave the house quick enough.
When you get to the barn there are 2 main places to watch. Watch the ladder you had to climb in order to get there, and watch the huge hole in the wall. Special infected will usually always come up the ladder and you can easily kill them before they become a threat.
When the tank comes have 1 or 2 people get onto the rafters, the tank will then try to climb up to get to you when he does this have all for people unload as much ammo as they can on him. Usually he will continue to fall and try to climb back up, but if he does get up then back off and try to kill him as you normally would.
When you kill the second tank just wait by the ammo stash. The reason for this is because the evac vehicle will be RIGHT underneath where you're waiting. Wait for the evac vehicle to pull up, throw a pipe bomb and rush to get inside.
These are the places I've found best to stay alive and get all 4 survivors to the rescue vehicle.
Personally I think that No Mercy, or Blood Harvest are the easiest levels to do this on. No Mercy is easy because you can trap the tank making him basically useless. And Blood Harvest is easy as it is the shortest trip to the evac vehicle.

I got this achievement by playing on No Mercy in single player on easy. This is probably the easiest campaign to do this on. Any other campaign it is possible but going to be harder.

There are no Hero Closets in the finale once the vehicle has been called in. So from that point on, you must watch each others backs. You will have to deal with a wave of Infected, A Tank, a 2nd wave of infected, and a 2nd Tank in that order. From here the vehicle will arrive in its designated location. You all must run and get on board, because the infected will constantly spawn and multiple tanks CAN show up.
