Stomach Upset
All Survivors complete a campaign without being vomited on.

How to unlock the Stomach Upset achievement in Left 4 Dead - Definitive Guide
This achievement is extremely frustrating even when playing on the easy difficulty.
The description says that all survivors must complete a campaign without being vomited on. However, it neglects to tell you that no survivors can be exploded on either. So you will need to complete a campaign without coming in contact with boomer bile at all. Only shoot them when you and your team are not in close proximity. If one does catch you off guard, remeber to melee, this will knock boomers off balance and give you more reaction time. Also, all four survivors must finish. You can't kill the computer players and finish by yourself. If your luck runs out and someone gets covered in spew, you can't restart from the last checkpoint, you have to exit to the main menu and start the campaign again.
I found Blood Harvest to be the best campaign to complete this. I also played with computer controlled teammates as they will be able to take out boomers hiding in the brush before you can even see them. Another trick is to turn on full captioning in the options menu. Now, during gameplay a message box will appear on your screen showing the dialogue, and more importantly, special infected. These messages will warn you of boomers long before their sounds become audible.
During mayhem scenes when you alert the horde, or just during infected attacks, simply melee and let your teammates kill the zombies while you keep an eye out for boomers. At the finale, i stayed to the left of the front door of the house you call for rescue in. Here you are a good distance away from the action, and will be able to pick off boomers before their throw up becomes a problem. Again, let your teammates battle the infected while you make boomers your top priority.

A very good solution. I got the achievement thanks for the help. Oh and just a side note I found that having 2 AI and 2 humans was the best strategy because 3 AI can be a bit too dumb for their own good.

I got this achievement without even going for it. I was trying to finish the crash coarse campaign on versus for that achievement. So i was doing split screen therefore there where no special infected so it was impossible to get vomited on. hope this helps people having trouble with it.

This and Nohing Special are not as hard as they sound. Set up Dead Air on easy play until you get to the begining of chapter 5. ( In safe room, Dont open safe room door.) Return to lobby. All prior special attacks are gone. Start at chapter 5. Don't get hit by any special and you will get Stomach Upset and Nothing special. To ensure you do not get hit, Hide behind the plane you board at the end, on the hump, on the border of the map. When the tanks come move foward, right agaianst the plane and it will get stuck,(shoot them from under the plane) get on the plane after the 2nd tank and bam, two achievements easy.

If you haven't gotten this achievement yet - do it on the new Crash Course DLC campaign. It's a shorter campaign and, thus, fewer opportunities to get spewed upon. I got it with little trouble this way - especially if you follow the instructions others have given in their solutions.

I did this in Crash Course while playing easy just to get kills and when i finished it showed up. What I mean is, its so easy in CC that i got it without even trying. Like enilson4 suggested, If you're having troubles in the others campaigns, try CC and you should get it easily.

Oh wow. That is very easy to do. I was surprised how fast I got it. Thanks for the tip.

The way I did this (accident) was versus mode, two controller (offline), running through the whole campaign from start to finish. Because the 2nd controller is infected, and you don't spawn with it, or you don't use the boomer, when you finish the campaign, its impossible to be vomited on!
The Nothing Special achievement might still work on this, but I haven't been able to confirm cause the stupid stupid AI keeps getting tagged right before we get to the truck.
*Note* To do it this way, you CAN use system link, but you absolutely cannot be connected to LIVE! it will not unlock if connected, so stick offline for this.

This achievement will drive you up the wall. i suggest a full party of four people, as the CPU has a bad habit of shooting boomers when they are too close, and the splatter will deactivate this achievement.
I found that Dead air, due to its wide spaces and lack of bushes to hide in was the best to do this in. Also putting full captions on helps, as it will say of any infected sounds, even if you yourself cannot hear them. Shotguns are reccomended - at range two shots will kill. on easy, three punches will kill a boomer, so if you are close up, be wary of how many times you have punched it.
Also, if one of you finds themselves vomited on, vote for RETURN TO LOBBY. If you select "Restart Campaing", It does not unlock. Go figure.
Either way, happy hunting.

First of all you should make sure that FULL CAPTIONS are turned ON.
Then pick whichever level you are comfortable with (I suggest Death Toll or Blood Harvest as there aren't really any corners the boomers can hide around. Blood Harvest is probably the best to do this on.)
EDIT: I would suggest getting yourself familiar with the Crash Course campaign if you bought it. Being it is only 2 chapters you have a much shorter campaign to worry about this. I have not actually confirmed if it works or not, so if anyone does go ahead and leave a comment.
Set the difficulty to EASY so you can rush through without getting incapped, and shoot any zombies that are slowing your teammates down without worrying about friendly fire. You want to rush through so that you will have less Boomers to deal with.
Rush through the level as FAST as you can. If there is a warning about a boomer STOP and keep an eye out for it so you can kill it without coming into contact with the bile. Having at least 1 or 2 bots may help with this as they can find Boomers very easily even through all the trees and bushes, but it is possible with 4 good human players.
Once you see the boomer be careful that no-one comes into contact with the bile, if any one gets vomited on AND/OR exploded on the achievement will not unlock.
After the Boomer is dead start rushing as FAST as you possibly can again so you can cover more ground without worrying about another Boomer spawning and ruining the achievement.
For the finale just keep a look out where they might come from, doing a level your comfortable with will help as you will know where they usually come from. I suggest Death Toll because of Jesus Rock, or Blood Harvest for the barn.
If you go to the barn in Blood Harvest make sure someone is watching the rafters and someone else is watching the hole in the wall. I suggest someone always watching out for the boomer regardless of whether or not the tank is coming. If you're at the finale and lose the achievement it is so much more disappointing (as I learned first hand)
And don't worry if you mess up, you probably wont get it your first try, personally it took me 3, and I got it on Blood Harvest in the barn, just watch the ladder and the hole in the wall.

Start up any campaign on easy (I did it on No Mercy since I was extremely familiar with it from Verses - Blood Harvest seems a favourite since the bots can see through the trees) with 3 bots. Make sure to turn on close captioning too. Just run through the levels as fast as you can, but when you get a boomer warning slow down and back into the corner of a large open area and the bots will usually take care of the boomer.
For finales, go outside to somewhere open. The bots will always kill the boomers before you can see them.

There's the legitimate way of getting this, then there's various glitches you can use if you don't want to get it in a noble fashion.
Legit: I would recommend playing a campaign you know well, in Easy mode, with four human players. Move swiftly with full captions on and when you hear a boomer's music cue, his distinctive groan or his caption, avoid him. Remember that meleeing him back and shooting from a distance is fine.
Glitch: There are several, and I'll cover them in detail.
1) Play all the way through an Easy campaign without worrying about boomers. Dead Air is probably the best one to do it on. It doesn't matter at all if you get hit by boomer bile, and you can just play through without worrying about it. You have to have four human players.
Now, when you get to the finale, DO NOT open the opening saferoom door and vote to return to the lobby. Now, in 'Edit Game Settings' change it to chapter 5 (which should be the finale) and start it up again. The reason this works is because the game acknowledges the fact you've played through the whole campaign, but it says you never got incapped, never healed, and so on (so you can unlock several achievements at the same time using this). Quickly rush through to the end of the finale, where the actual finale 'event' take place, and you should have no Special Infected spawns if you're quick enough.
There is actually a glitch to beat every finale without so much as killing a single Infected. On No Mercy you can let zombies break down a door and then go into a room where the AI can't see you, on Dead Air you can jump on top of the plane by moving one or two carts with propane tanks, on Blood Harvest you can actually just stand and melee a tree and everything will immediately die before it reaches you (!) and on Death Toll there's a rock you can sit on. Crash Course, as far as I know, has no finale glitches. But I'm not going into how they work and not saying you have to use them. It has little relevance, just find them out through Google or YouTube.
I hope this helps.

Play versus with another control, have the other control be the infected but never spawn, the player will be the only infected to spawn in the whole campaign, I got this plus 5 other achievements at the same time!
Tip #1: Avoid all boomer bile. This means you can't let a boomer puke on any survivor OR explode on you or your teammates. All survivors must survive to unlock the achievement, and this can be done in any campaign.
Tip #2: This achievement requires a good bit of luck and even more patience. Unless you can find a committed and skilled group of players to attempt this with, solo is the way to go. The AI-controlled survivors are badass at shooting special infected, but sometimes they will be a little stupid and shoot a boomer at point blank-range. This is where the patience comes in.
Tip #3: As was mentioned in the above solution, turning the captions on will often let you know there's a boomer nearby before you can even hear it.
Tip #4: Think before you shoot. When a boomer or any infected suddenly pop out in front of you, it's instict to pull the trigger. You must retrain your insticts to MELEE a boomer in front of you, which will save you from being exploded on. When it stumbles backward, take a few steps back, then shoot it with whatever you please.
Good luck! Hope this helps.

Easiest way: Crash course DLC. Play with just you and 3 AI's or You, a friend who is very good at the game and 2 AI's. Just be quick and run through the campaign but make sure your killing all of the single zombies as you run. Also quickly save your teamates from being attacked from hunters etc. Stay out in the open and turn up your sound to listen in to where they spawn. Dolby 5.1 digital surround sound helps ALOT. (Good TV's/TB X1s and up) Melee if they are close and shoot if they are far its that simple I actually got this without trying. Hope I helped.
Ok, So If you have the crashcourse dlc this will make it easier, if you don’t just use dead air, you also want to put subtitles on in main menu>Options>Subtitles>On then just start up crashcourse or dead air, the subtitles will tell you stuff like [Smoker Warning] [Hunter Growls] etc And will tell you if a boomers around, And try to speed run in open areas and go slow in tight areas, also make sure your group is with you. You would always want to kill a boomer before the a.i Do. Hope this helps..
Start the No Mercy campaign on easy. It's recommended that you do this with 3 other players so you don’t have to worry about the computer controlled characters getting vomited on. You must be careful when turning corners, there might be a Boomer waiting for you. They only take one shot from a shotgun to kill, so use it wisely. If a Boomer explodes on you, it will count as being vomited on. Remember to listen for the Boomer, it makes a distinct sound that you should notice right away.

If you're playing with bots, it's best to play with a group from the Fateful Flight expansion on Easy difficulty. Don't run from the bots, be with them and go slowly, because in this mode they shoot more accurately and can take out the enemy at a greater distance.

Easier to do with a second joystick.
Create a version lobby, then select Survivors on the first joystick, and Infected on the second.
The only special infected will be under your control on the second joystick.
We take the first joystick and quickly go through the company.
Just remember that on the second joystick they sometimes let you control the tank, and so at the moment of controlling the tank, the so-called “Tank Control Line” will decrease, if after this line, you do not have time to reach the survivors and take damage from them, then the computer itself will take over control of the tank.
So, while driving a tank, you need to quickly kill it, just expose it to bullets or throw it off the roof.
Also, there is one more, quite important point - when you control the tank on the second joystick, at this moment, the computer will control your special infected, which you had in control, before the tank.
So, be extremely careful!
You need to quickly kill the tank so that the computer at this moment, controlling your special infected, does not have time
harm someone.
After killing the tank, under the control of the second joystick, the previous, special infected one will again be taken over.
Here is a video of the walkthrough, with the second joystick:

Also, this method can be combined with the achievement – Nothing Special / Nothing special
