The portals can be found by going left in the main portal room and taking the lift up to the surface. DC and Lego Movie can be found on this level a bit to the right of the lift and Lord of the Rings can be found by going up the ramp immediately in front of you you as you come off the lift and to the right.
Once you land in the final world the achievement will pop.

If you don't know how to access the hub levels, after completing the first few story missions a doorway should have opened up on the left hand side of the main portal room. Go through it, take the elevator up, and you will find yourself in a room with gateways to each hub level.

Visit the HUB portals from the elevator where you got the "Home, Sweet Home" achievement:
- DC Comics - Home to Batman
- The Lord of the Rings - Home to Gandalf
- The LEGO Movie - Home to Wyldstyle
Once you land in the final world the achievement will unlock.

The trophy description is a little misleading because you don't have to visit the physical houses of each character, only their worlds. To get to each of the HUB worlds, from the main portal, head to the left and take the elevator up. Head into the Lord of the Rings, DC Comics and LEGO Movie HUBs and on entering the last world the trophy will unlock.
