If the achievement doesn't unlock, just go to a level of the game and rescue him once again

You need all of them in the story, the Gwenpool levels and the city areas. Just look for the little Stan Lee heads on your city map. You can zoom in and out with the triggers to make it easier to see them. Don't forget to switch layers so that you can see Attilan and Knowhere.

You will need to rescue Stan Lee from 50 different perilous situations. He is indicated by a Stan Lee head icon on the map.
The breakdown of where you'll find him is as follows:
Story Missions: 20 (One per level)
Gwenpool Missions: 10 (One per mission)
Manhattan: 2
Wakanda: 2
Xandar: 2
Man-Things Swamp: 1
Nueva York: 1
Sakaar: 1
Asgard: 1
Old West: 1
Hala: 1
Egypt: 1
Lemuria: 1
Manhattan Noir: 1

To unlock this achievement, you need to rescue 50 Stan Lee in Peril characters in the game. There’s 20 in the story levels, 10 in the Gwenpool missions and 20 in Chronopolis. Most of them can only be rescued in Free Play and certain character abilities are required. When you’ve rescued them all, you can purchase Stan Lee for 500,000 studs.

Note: Stan Lees rescued in DLC missions do not count towards this achievement. If this does not unlock after rescuing all 50 Stan Lees in the base game, load up any base game mission in Free Play and when it starts, exit and save the level so the results screen appears. The achievement then should unlock.
