This is unlocked for completed all 5 levels in Film 4, which is called 'On Stranger Tides'
The 5 levels in this film are:

3) White Cap Bay

5) The Fountain of Youth

This achievement can be missed as it happened to me.

While playing with someone, when I got to the Blackbeard fight, the game kicked me out and made the other player priority. This happened about 4 times during the fight. So when the fight ended, no achievement popped.

I went back and played through the Ch 5 again by myself and luckily the achievement popped for me.

Well only player 1 can get the achievements, so under NORMAL circumstances it isnt missable

I had the same thing happen to me. I was playing coop with the wife and every time we stunned blackbeard and gave the chalice to the woman i got kicked out and had to press the start button to join game again, unfortunatly it does this on the last heart so you cant rejoin to get the cheevo.
As you say the primary profile gets the cheevo so both the wife an i missed out.
Perhap you might wanna add this glitch to your guide and just warn peeps that its possible and prob best the ensure you fight blackbeard on your own.
Other than that good guide +ve from me.

Same thing happened to me playing co-op. Quite a pain in the butt to go back and do it again. There's a few glitches in this game so far. Unacceptable if you ask me! Nothing wrong with this guide of course, just venting.
