This is the hardest achievement in the game! The achievement has a hidden unlock requirement all the tasks must be completed by the same operative that you play on the last round
If you want to get this in a normal game, I wish you the best of luck. I recommend grabbing a friend & boosting it. If you follow the steps shown in my video you should have no problems boosting the achievement. Post a comment if you have any questions & I will do my best to help.

man this achievement is evil i knew there was something messed up about it thanks a lot man would not of got this without the guide biggest +1 from me

Thanks for the info, I could've sworn I've done this multiple times, but I had no idea you had to capture all objectives with the same character on the last round. That's pretty insane.
