There are 5 Trinity Sanctums to find in Lies of P. To unlock them you will need a trinity key which you get from solving riddles on the ringing telephones throughout the game.
Key 1 - Workshop Union Entrance (To the right before the bridge leading to the factory) I answered monster which the riddle master said was wrong but I still got the key. Not entirely sure if it matters.
Door 1 - Near Veniginis work control room stargazer.
Key 2 - Malum District (In front of you after climbing a ladder) Answer: Candle
Door 2 - St. Frangelico Cathedral (You'll reach a spot where you are dodging electric balls. At the end to the right is a ladder you can take, follow the path to the door.)
Key 3 - Grand Exhibition (You'll see the phone a long the path shortly after killing an elite who bursts through a gate.) Answer - Egg. After the phone go down the next staircase and turn the statue on the right twice to get the key.
Door 3 - Estella Opera House (before going into the boss room jump down where the flaming chandelier is and you should see it.)
Key 4 - Krat Central Station (When you go back to krat central station the phone is a long the waterfront not far from this stargazer.) Answer - Ergo
Door 4 - Krat Central Station Lobby (Fast travel to the stargazer go up the first flight of stairs and its on the left)
Key 5 - Relic of Trismegistus Entrance (near the boss instead of jumping down stay on the path upwards and you'll see the phone) Answer - No
Door 5 - Arche Abbey Outer Hall (Deep into the path you'll work your way back inside the castle and there will be multiple flame thrower enemies. The door is all the way at the bottom.)
When you reach the final Trinity door make sure to exhaust all dialogue options with the riddle master. He will give you a special relic you can take to Venigni which will unlock the achievement for talking to him.

I answered 'Yes' for the 5th riddle, got the key straight up without having to open the box.

- The progress is kept through NG+ -
If you failed the last key and can't find a red apple like the half of us, you can start a NG+ and only open the door(s) you missed on your first run, you still get the achievement, no need to do the whole thing again.

I skipped the one in the cathedral but I’ve answered all the riddles correctly. Went back and it says I need a key. Any fix for this or is it just going back in ng+?

Did you retrieve the key from the statue in the Grand Exhibition? Answering the question correctly doesn't automatically give you the key you have to go get it from the statue. I missed this one my first playthrough.

I organized this one based on key locations and sanctum locations. Since you can use any key in any sanctum (except for the last) that way you can approach it as you see fit :)

Telephone location 1: Krat City Hall Courtyard
- This one is pretty hard to miss, but when you pass a gate heading into the factory, walk past the bridge to find the first phone and answer it, getting an introduction to the King of Riddles
- Answer “human” and you’ll get the trinity key
- After fighting through the miniboss in the middle of the square with some guys throwing decay up top, just around the shortcut, head up a ladder to find the telephone ringing
- Answer the riddle with “Candle”
- Shortly after opening up the shortcut, keep on going and you can find the telephone on your path
- Answer the riddle with “Egg” then keep walking past the telephone to find a little area with a bunch of statues that really stand out
- Rotate them around so both of the ladies are pointing towards the middle, and it will open up a little window giving you the key
- Post earthquake, head to this stargazer and instead of heading to the parade puppet’s area, turn around and follow the path
- You may recognize this telephone because the first time around it was inactive, but this time it will work and you can answer another riddle
- The answer here is “Ergo”
- Just before dropping down to fight the black rabbit brotherhood again, instead keep running forwards and you can find the phone just up past some puppets
- Answer the phone and the king of riddles will give you one final riddle asking you if you’re a killer or not
- If you say you are not, then you get a surprise box, but if you say that you are then you get the chosen one’s trinity key
- If you open the surprise box, it’ll inflict a random status effect on you, but you get the key all the same, so no sweat what you answer here
- Assuming you’ve got all of the shortcuts unlocked, go up the reverse platform stairs on the right and all the way up to the area where you fought the big guy miniboss behind the caged door
- Head up the ladder at the end, and right down the hallway you’ll find it

- About halfway up the tower, there is a giant electric ball that will come down at you Indiana Jones style
- Go to this location and find a ladder around where the ball drops
- Simply go to this ladder, dodge the NEXT rolling ball and then find the sanctum on the left
- items: Black Cat’s Amulet, Monster Sweeper’s Hunting Apparel, Full Moonstone
- After opening up the entrance to the boss, this time just drop down into the pit, and if you turn around you can find the doors just hanging out right there
- items: Belford Superior Radiation Converter and Quartz
- Post earthquake, when you get back into the central station, instead of going straight, look to the left and you’ll see some stairs descending downwards towards the sanctum
