How to unlock the The Story of the Refined Old Lady achievement in Lies of P - Definitive Guide
Important Note - Be sure to talk to every NPC when you see their Icon in the teleportation menu, this means they have new dialogue, and this will advance their storylines!
After unlocking the Gold Coin Fruit Tree, you will see that Polendina (Antonia's Puppet Butler) wants to talk to you regarding Antonia's condition. At the end of chapter 7, after defeating "Champion Victor", return to Hotel Krat and you'll see that Giangio wants to talk to you. He will be able to create a cure for Antonia for 1 Gold Coin Fruit. After getting the cure, talk to Polendina, and choose the option to administer the cure, then talk to Antonia.
In chapter 11, after defeating "Laxasia the Complete", head over to Hotel Krat. Talk to Polendina, and go upstairs to Geppetto's office where Polendina was. You'll find that she has passed, and has left behind some items on her wheelchair. That is the end of her story and the achievement will unlock.

I haven't been able to get any items left in the wheelchair both in my initial play through nor in my NG+. Both times i gave the cure to Polendina and told him to administer the Cure for Antonia. I don't know if it is a bug or i'm missing something?

@Z1PPYZ3NO - Sorry to hear about that, did you talk to them whenever they had new dialogue, when their icon was on the teleport menu? I’ve been told people have been locked out of the other NPC quest lines as well by missing these interactions.
But for this particular one, it may be mandatory to talk to Polendina first as I mentioned, as he tells you that she has passed. It’s kind of finicky of having to talk to a certain NPC first and then the other.

**MISSABLE**
If you tell Polendina not to administer the cure to Antonia, then you won’t be able to unlock this achievement. Here’s how you get it. After you have beaten the Champion Victor, talk to Giangio up in his room in Hotel Krat, give him the Gold Coin Fruit and get him to craft the “Cure for Antonia.” Go and talk to Polendina and ask him to administer the cure.
Then continue through the story until you beat the Laxasia The Complete boss. Once you have, head back to Hotel Krat and talk to Polendina. Then head to Antonia’s wheelchair, in her study, and read the letter. The achievement will then unlock.

Missable trophy . If you ask Polendina not to administer Antonia's medicine, the trophy will be blocked.
At the end of Chapter VII, after defeating the main boss, Champion Victor, return to the Krat Hotel and go upstairs. Talk to the Merchant in the room near the tree with gold coins. Give him some gold coins so he can make a “Cure for Antonia.” Next, go downstairs and talk to Polendina, convincing him that Antonia takes the medicine. After this, talk to Antonia in front of her portrait and she will say that she is feeling better. Then continue following the story until you defeat Laxasia the All-Perfect. After the victory, return to the Krat Hotel and find out that Antonia has died. Talk to Polendina and get the trophy. Be sure to go up to Geppetto's office to pick up the rewards Antonia left on her wheelchair.

I'll add a little to the tip above.
After killing the boss, first talk to Polendina, he will tell you that the medicine can be prepared by the alchemist who is next to the tree (without talking to me, the opportunity to ask for medicine did not appear) and then do everything as the person described above☝️
