To unlock this achievement, you need to equip the following items all at once:
- Ninja headband
- Nunchaku
- Trampoline
- Shuriken

Only the Ninja Headband, Nunchaku, and Shuriken are needed, “shield” item (trampoline in this case) doesn’t matter. I had the Tower Shield equipped when it unlocked.

I would recommend just leaving the 3 x Costume related Achievements until near the end rather than seeking out the items specifically, you need to make friends with everyone anyway and buy everything so by doing that you will have all the Costume items that are used for the costume achievements, so I left these until near the end when I'd gathered them in as part of the other collectables achievements.
For Hidden Scale Clan equip:
Ninja headband
Nunchaku
Trampoline (I equipped the Trampoline as shield but some gamers are saying it popped for them with any shield)
Shuriken
