How to unlock the Broken geography achievement in Loop Hero - Definitive Guide
So this took me 16 loops on the 2nd expedition to achieve.
doomedtolive has very nicely laid out everything you need to know to do this.
However if you would like someone to figuratively hold your hand, that's what im here to do.
Deck Build
The general idea is that we maximize the amount of "living soul" enemies we spawn and that when they die they die in the area of effect of the Battlefield, that way they have a chance to become 'Ghosts'. Upon killing a 'Ghost' it has a chance to become a 'Ghost of a ghost' and upon killing that it has a small chance to become a 'Prime Matter', which gives you 3 random cards that we will be using to complete the achievement.
I played as the Rogue so your results may vary if you're playing alternative classes.
Building the map is very basic. Place your Arsenal. Stagger Villages across the loop as you normally would to keep your HP up. Build Suburbs on the left of the map and ONLY Thickets(Forests suck) on the right, both building towards the center. As you continue to loop, keep expanding your Battlefield influence so that it covers every single tile in the loop(I had about 11 at the end). Anytime you get a Grove card, slap it down where you have Battlefield coverage as its your primary generator of Ghosts and will occupy 90% of your road tiles. Whenever you get Vampire Mansions, use them to turn Villages into Counts Land and stagger extras to cover tiles where you have Groves. I had 6 Counts Lands at the end and had no problem sustaining my HP. Use the Oblivion cards you obtain to nuke undesirable tiles like Bandit Camps and those lame Forest Villages that generate when you place Forest/Thicket tiles.
Things to look out for:

Things to look out for:
- Card-Sharp trait(10% chance to not consume a card when placed) - Extra cards generated DO count towards the achievement.
- When the map is starting to get close to being full remember to leave tiles open for the cards you will get as drops from the Prime Matter. That means making sure a couple road tiles are empty and you haven't filled the map with Suburbs and Thickets entirely. You will hate yourself if you have no tiles to place the cards on and you have to watch them disappear into the abyss.
- Prime Matters 6th attack is guaranteed to kill your hero. So drop that DPS. (Hunter's Mark is great for snowballing large groups).

Excellent setup - worked for me on the second try.
Used the same - without the Arsenal - to get the Fence Achievement and also good for farming Orbs of Expansion.

Completed first try following this guide, I recommend using Skinner's Knifes in the tools supplies to increase damage to Ratwolves

Thanks!
Popped on loop 14, 2nd Expedition, using Warrior. Card-Sharp trait didn't appear for me, sadly, but it didn't stop me from getting the achievement.
Lost 2 cards because my road was full and I hadn't Oblivion. Then I started to use oblivion to clean some road pathways.
The cards I got: 2 Spiders Cooons, 1 Maze of Memories (all rounded up so it didn't spread the map), 1 Treasury, 2 Cemetery, 1 Storm Temple, 2 Chrono Crystals, 1 desert.

This is an RNG heavy achievement.
THIS IS NOT CUMULATIVE. MUST BE DONE IN A SINGLE EXPEDITION
What's needed:
This monster is generated by a leveled up battlefield after enemies with souls are killed on it and become a ghost. (20% chance) They have a chance of spawning as the third revive of any ghost.
When killed you get 3 random cards.
These can include ones in your deck already and those won't count. There's also a 33% chance of getting a golden card.
The reason to play on expedition 2 is that the monster won't have the perk on level 1.
Get some battlefield tiles out and place a monster spawn within the aoe, and then just run loops hoping for the spawn and then that the cards you get aren't dupes. Make sure to play them when you are given them. Just getting them in your hand doesn't count.

Cards can be repeated, and I have three libraries that still unlock achievements. But the library and the abandoned library are only counted as one.
It's not about counting the number of cards in the slot. You can place a card as soon as it appears (of course, leaving it in the slot will cause it to be erased)

I don't understand. Are you distorting something I said? And if you are talking about the bookery card, the cards they swap out do not count for this achievement.

The only way to get this achievement is through spawning Prime Matters. Prime Matters have a small chance of spawning from a Ghost of a Ghost which likewise have a small chance of spawning from a Ghost. In order to spawn ghosts you must place down a Battlefield and kill any creature with a soul in its radius. It is recommended to have as small of a deck as possible in order to maximize the potential drops of cards not in your deck.
You also have to do this on Act 2 or beyond so that the Prime Matter has the ability of dropping a random card, though it is recommended against doing it on Act 4 as Prime Matter might have an ability that can one shot kill you. Since a bit of luck is required it is recommended to do this later on in the game after you have levelled up your town and have good gear equipped like Vampire Chairs in order to survive running the loops. Make sure to leave some space open to place the cards.
I personally did this after I had levelled up my whole town and I did it on Act 3 with the Rogue. It took about an hour and a half to get, my card loadout was Oblivion, Battlefield, Grove, Spider Cocoon, Suburbs, Forest, River, and Village.

Time needed to complete this achievement is mostly RNG based, so I am sharing the full run for 'Broken Geography' achievement and how I got it.
Steps are same as wrote by the other two solutions here, but in video form for anyone like me who wants video guide.

1 - The second level of the expedition (the third is also possible)
2 - Mandatory maps such as: Grove, Battlefield, Cemetery and Swamp (3 swamp spots will give the Witch Hut card, provided that the herbalist is pumped out in the camp)
3 - We need to create the most convenient farming circle. Place monster spots and cover them with the battlefield map area.
4 - Don't play too many cards, keep an eye on the boss' spawn level.
5 - Don’t take a lot of cards, take exactly 7 minimum cards to start an expedition (Grove, Swamp, Battlefield, Cemetery, Oblivion, Rocks (to create a large rock), Village).
All these conditions will lead you to what you want. By killing monsters with souls in the aura's area of effect, ghosts will appear on the battlefield. So we need these ghosts, which will then become super ghosts and, in the third phase, cosmic entities. This very essence gives random cards when it is killed, even those that you do not have in your deck. By the way, this ghost has an imbecile skill that can one-shot you, but it is available at the 4th level of the expedition, which is why we take 2 or 3.
All cards that you receive without having them in the deck must be exposed. You cannot delete them, and it goes without saying that all 10 cards must be played in one expedition.
Everything stated above is not my personal experience, but taken and retold and translated from the description of another person.
Believe in your luck and find 4 hours of free time as this is an achievement based on endurance and your luck.
