This achievement is for completing your first run.
This will most likely consists of
- Sunken Village
- The First City
- Chapel of Revenants - 'Gwen, the Matron' Boss (Boss Room only)
- Opulent Sanctum - 'The First One' Boss at the end of this area
After defeating the First One, Iphis awaits above to take you back to Sanctuary. When you get there, everyone will be stood around the centre crystal Relic - simply attack it once to complete the run and get a victory screen.
A few starter tips

Any tips for the First One boss or Gantley the Usurper on the Sunken Village?
Also, any tips on best weapons/spells?
I found Gwen to be very easy but these other two are a pain.
Gantley is mostly difficult I think because you have no upgrades on the first level and only a few potions.
Edit: I'm getting better at parrying which makes Gantley a bit easier. Also, I realized he's only there because of a modifier I have on that makes him guard access to The Divide exit.
