This achievement requires you to defeat the boss in the 'tutorial' called "Gantley, the Usurper". Parrying is king here.
After defeating him and progressing you'll reach the elevator shaft and be told its currently out of order. Soap will then kick you down it, killing you and starting the game properly.
If you happen to die fighting him during the tutorial you can select load game on the main menu and select a new save slot to try again.
