Lords of the Fallen (2014)

At the start of the game walk up the stairs and turn right and you should see it.

Your first opportunity for this is at the very start of the game in the monestary. After the initial fight tutorial, walk up the stairs and talk to the wizard bloke you entered with. Facing him, and just to his right is your first note. Pick it up (hold Y) and the acheivement is yours.

If you somehow miss it, don't worry as there are another 3-4 on your way to finding the key to continue.
