This achievement wants you to collect 25 Human Skulls. You get about 5 on the graveyard and a lot of them in the catacombs. Don´t worry you find plenty of them!
SPOILER! (Has nothing to do with that Achievement because you just have to collect 25 skulls, but you need them for that NPC to get an special Item).
if you enter the Rhogar realm for the second time Crippled Rhogar has changed his location to the cliffs. Look at point 9 on this map:

Dieser Erfolg verlangt von euch 25 Menschenköpfe zu finden. Ihr bekommt ca. 5 am Friedhof und einen Haufen in den Katakomben. Keine Sorge ihr findet mehr als genug davon.
SPOILER! (Hat zwar nichts mit diesem Erfolg zutun da man die 25 Köpfer nur sammeln muss aber man benötigt sie für diesen NPC).
Wenn ihr das zweite mal in die Rhogar Dimension reist hat der Verkrüppelte Rhogar seine Position gewechaselt zu den Klippen. Schaut auf dieser Karte unter dem Punkt 9

are there skulls in any places besides those two locations?

Yes. i can remember i found one or two in the cellar and in the abadoned temple. You find more than needed in the game.
