How to unlock the Only after Disaster achievement in Lords of the Fallen (2014) - Definitive Guide
Sliktor has an excellent review on the boss fight and its mechanics.
Let me give you some HINTS and TIPS on how to fight him.
Before you take his first health bar he will not summon any supporters so try to get a hold on his moves. Sliktor explained his moves very well.
Please keep in mind that when you dodge roll you are invincible to physical attacks so it is a great way of avoiding his attacks. (You can take damage when you stand up from a roll so heavy characters might suffer from this)
After his 1st health bar depleted he will keep doing his attacks but also summon minions.
I advise that you quickly take care of the minions before paying attention to the Boss as 2 minions plus the boss can be overwhelming.
TIP: Minions do take damage from the boss. Any bomb or physical attack they walk into will dmg them. If you are close to deplete another segment of the BOSS`s life do so and use your clone to attract the minions. Then run to a shrine while the boss is summoning his "ultimate" magic which will also take care of the minions which are hitting on your "clone".
I was a lvl 26 Rogue when I defeated the BOSS. I used the clone ability while the boss was charging his magic to attract minions & used my STAB ability to quickly get ride of the minions. I did not spend magic on damaging the boss as getting rid of the supporters quickly is more important.(3 of them can kill you in a few seconds giving you no time to heal)

This is the third boss of the game (doesn't have a shield!) and isn't very difficult overall. Here's a quick run-through of his moves and couple suggestions on when to attack.
First and foremost, every time you take out a quarter of the boss' health, he'll kneel on the ground and start waving his arms about, raising pools of blue mist all over the graveyard. Then, after a few seconds, the air explodes and you take a couple thousand damage. The only safe place to avoid this are in the lantern lit shrines along the sides of the graveyard. Stand in one during the explosion and you'll be safe from the attack, though not from any minions that may have chased you into the shrine.
He has a melee attack where he swings around a scythe once or twice when your near. You can get in a couple of good hits after the second swing by staying just out of reach and moving in quickly.
When you're up close, he will grab the scythe with both hands in front and drive it into the ground, sending out a short shockwave that'll stun you. He recovers quickly with a follow up melee attack. If you get hit by this and can't recover in time to dodge the follow up attack, circle around to the left to dodge the second swing and give yourself an opening.
If you start getting too far away he'll rest his weapon on the ground, reach into a pocket, and throw 4 four skulls at you. These skulls bounce twice once thrown and cause a small explosion each time they hit the ground. If you get up close while he's reaching in his pocket, he will (sometimes) cancel the attack and try to melee you instead.
Regardless of where you are, he will occasionally hit his scythe to the ground, then follow with an overhead swing, burying the head of the weapon in the ground. While he tugs on the scythe, the ground beneath you turns blue and soon thereafter a black spikes stabs out of it. There are two in quick succession, then a third that is delayed. Keep moving to avoid the spikes. If your close to him, you can get in a quick hit or two between the second and third spikes, just be prepared to dodge right after the attack.
After the Worshiper loses his first bar of health, he'll occasionally summon minions by pumping his fist into the air and "dragging" it down. He takes a long time to do this and it's easy to get in a lot of hits. The minions start as a blue cocoon of bones that can be destroyed in one blow if you reach them in time, otherwise the quick dual fist weapon enemies will spawn. He only summons two at a time, but can summon more regardless of how many are still alive.
If you can get him to hit one the shrines along the sides of the graveyard, it will explode and stun him for a few seconds allowing you to get in some good hits.

Great video guide! Multiple things to say. The fact that you use your gauntlet to kill summoned minions is effective; however we see it twice you had to deal with some. I chose not to wear any armor for this fight making me super fast : when he summons them, I run to their "eggs" and kill them instantly with none being able to break through. As for going into circles, around the boss, I, sometimes, got closer to him instead, making him attack me thrice with its weapon without planting it on the ground... Once he's triggered his melee, jump back and use your gauntlet. Once again this is what I did but yours fight was efficient.
