Tortured Prisoner is a companion that can be summoned to defeat certain Bosses, they are available to help you as soon as you free them from their prison cell.
Once you acquire the Skyrest Bridge Key and progress inside Skyrest Bridge there will be jail cells around knowing you are in the right place. Go into umbral and find the 3 pods to soulflay to unlock the umbral door that has the item Searing Accusation. Bring the item back to the Tortured Prisoner where you found her in one of the cells in Skyrest Bridge where you soulflayed the 3 pods.
After freeing the Tortured Prisoner from her locked cell, you will find her at the top of the Skyrest Bridge, outside the Hub. This location is near the arena where you fought Pieta, She of Blessed Renewal, and also near the fallen giant. Exhaust her dialogue to progress her quest.
To progress the quest of the Tortured Prisoner, you need to give her the Giant Eyeball that is acquired after defeating the Spurned Progeny. Then, retrieve the Noblewoman Set in the marketplace at Upper Calrath. Equip the full set and return to the Tortured Prisoner at the arena of the Spurned Progeny. This is a really poorly or scummily designed quest. The quest locks you out even if you don't fight the Sundered Monarch if you kill the seer before the monarch. Once you get the cloth use a moth and get back to Sophesia or you'll have to do a whole playthrough just for this achievement.

- Go back to the boss arena where you fought the Spurned Progeny and give the Tortured Prisoner the Giant Eyeball, and then talk to her.
- Purchase the new spells that she now offers.
- Rest, and you will find that she moves next to the beacon in Upper Calrath. Talk to her, and she will give you the Elegant Perfume.
