This Quest is pretty Simple, Easy to follow and hard to miss without running too far ahead.
I believe this quest is missable if you have access to 2 of the 5 beacons as Andreas will leave and drop the fief key where he is suppose to be standing next to the vestige after step 1. I'm not sure what trigger this interaction specifically as step 4 interaction might not happen if he does.
Edited - This quest is only missable if you don't complete it during your playthrough as I ran through my new game plus run and could still complete his quest, if you have already completed the game and want to do his quest skip to step 4
Step 1 - From "Skyrest" Vestige turn right and straight ahead is a door with some outside with some stair this heads towards pilgrims path by you can find Andreas the ebb standing next to the cliff talk to him and exhaust all of his dialogue.
Step 2 - After the Main boss Congregator of flesh found upon entering "Forsaken fen" return to "Skyreach" and talk to Andreas once again he will to right next to the Skyreach Vestige he will give you the fief key this is for much much later on in the game.
Step 3 - After beating the first few areas of the game you will need to go back to the spawn area to use the fief key on the door with the bell at the windmill vestige.
Step 4 - Once in the fief proceed through the area until you get to the vestige and carry on until you pull the lever to open the game, go through the gate and proceed until you reach an icy cave you will see an item on the floor (this is a gesture) CAUTIOUSLY go towards the item and this will trigger an interaction where Andreas will drop an ice block on it, THIS WILL INSTA KILL YOU IF YOU ARE NOT CAUTIOUS. this ice block allows you to continue in the game through the fief. In my NG+ run where I rushed the game and missed Andrea this interaction didn't happen could have been I was in umbral form.
Step 5 - Go back to Skyreach vestige and talk to Andreas he will have moved locations, from vestige turned around and run down the first set of stairs he will be on your left in one of the rooms exhaust his dialogue and continue through your game. (This quest cannot be missed after this point)
Step 5 - Proceed through the game after until you have acquired the Rune of Adyr in the abbey. From Skybridge run towards the Pieta boss room and running up the stairs towards the upper Calrath area/shortcut upon entering the door for the shortcut you opened previously go into umbral this will reveal a blue umbral imprint infront of you, use soul flay on this imprint and you will receive Andreas book.
Step 6 - return to Skyreach and give Andreas his book. if you have killed the lightreaper move to Step 8, if not kill the lightreaper in "Upper Calrath"
Step 7 - Die to the Lightreaper and summon Andreas into the boss fight
Step 8- After the Lightreaper boss fight proceed forward past the boss fight and Andreas will spawn in as a boss, defeat him and you will get his armour and you quest is complete good job cheevo unlocked.

It should be noted that Ebb doesn't help you in the light reaper fight. He leaves immediately hoping to collect your lamp after you die. I died to the light reaper the first time around and was worried because Ebb wasn't a summonable character. It should also be noted that unlike previous light reaper encounters you'll have multiple attempts to try and defeat him.

You cannot summon Andrea's into the light reaper fight. He is stood outside the arena just speak to him and he will enter but not as a summon. This means you can summon kukajin In for her quest line requirement.

To unlock this trophy, you have to kill Andreas of Ebb and collect his Book of Lineage. He is a sneaky operator and pretends to be an ally, but actually he just wants the Lamp off you. Here is how to deal with him.
