How to unlock the Inefficient Medicine achievement in Lords of the Fallen - Definitive Guide

Going to add another point of view/guide for these achievements. STViii already has an in-depth guide here so this will not be one as most things would just be repeated. This will simply be my advice after beating the game 4 times with all the different “OP” builds and crucibles completed. So let’s start.

First thing, do ALL your runs in New Game+0. Do not go into New Game+1. That makes the game much harder for no reason. Can do unlimited restart worlds.

3 different endings ranked
1. Umbral - By far the best ending to go for. Why? You can completely skip the Fief (Fallen Crow Boss, annoying to fight if you don’t enter umbral), Tower of Penance (annoying platforming in umbral) and The Empyrean (end game enemies)
2. Radiance - The default. Only advantage is you skip the Ellaine, however with the Crossbow build you should be beating bosses in under a minute or two. If you ever die, immediately close the game and it will respawn you outside the boss arena.
3. Infernal - Do not attempt, no vestiges makes it a pain to run back to all the beacons again.

-The only hard part about ironman is the withered health. You can attempt a playthrough with all modifiers sure however imo this should be your 3rd playthrough after getting all the quests/majority of achievements done. This is a speedrun essentially. I reached Lower Calrith in an hour. I personally did first nothing, second all except ironman, third ironman + related.
-Make sure you defeat the light reaper in the intro/on the first attempt. One of if not the hardest bosses so you can get that out of the way 5 minutes in.
-Unsure when Upgraded Loot becomes +10/+5. Personally reset the first time at level 147 and all drops were maxed out +10/+5.

Builds Ranked
1. Harrowers Crossbow + Aelstrix Runes + Eye of Numb Witch + Blackfeather Ring + another ring (personally went with crossbow ring). At 75 agility and 50 str this build decimates everything. Quick tip - how much withered health you lose per shot is dependent on how much ammo you have left. If you have 0 bolts left you take a lot of wither damage, if you have 2-3 bolts left (will be in red) you take minimal damage. Use this to your advantage.
2. Rad/Str Build - The best pure survivability build. 2 mana rings + mana regan runes and you can have permanent health regan spell on. Damage is much less than the crossbow build however. Can use Orians Judgment to dish out decent damage though. Plus health regan is all withered in this challenge.
3. Inferno Build - While you can dish out a ton of damage dual-wielding Grinning Axes or Magma surge it's just average at best. You have to get in close.

Make sure you activate all the shortcuts possible. Sucks to miss something then have to run back through an entire map.

That’s about it. Take your time doing any platforming and do not spend any excess time in the Umbral Realm. Majority of fights you can simply run by or pick off from distance before they even register your presence.

All three Ironman trophies can be completed at the same time, just don't turn on random enemies.
