Important: If you are planning on trying to get the "Collect All" achievements, do not enter NG+ after beating the game. There are no vestiges in NG+ so you can not fast travel around the map. (Developers mentioned they might add them because of the negative feedback)
Umbral eye of Ethryg is missable. If you go up the elevator after defeating the skinstealer boss before completing Byrons side quest you will be locked out of his quest and cant receive the eye.
The other 14 are found in the enviroment, 1 of which is dropped from an enemy.
To check which ones you are missing open the inventory and go over to the Umbral Eye Tab.

Important to mention: NG+ now gives you an option to keep all vestiges before you start it. First paragraph of guide is no longer accurate.

There's some really inefficient pathing in this video. Especially for Dietr and Lydia the Numb Witch. I was only missing three so can't comment on the others. But I highly recommend watching the route in full before you start hunting. Odds are you will find a much better route if you have a bunch of shortcuts already unlocked!
