Simple achievement compared to other questline achievements. Put it simply, you have to find this character merchant in certain locations and you have exhaust he's dialogue in Dark Souls fashion. Just in case make sure that you will find him in these locations before killing Judge Cleric boss. Then kill Judge, obtain "Thorned Chalice" and that is that.
Important personal notes first;
This character is a merchant so be sure to check him once in a while for new gear towards all weapons and armor etc. achievements.
Make sure that you will speak to him before going for another vestige, he will not spawn in certain points if you move too far of the story. For example I advanced father in towards Judge Cleric boss route (Leprosarium, Vestige of Brother Jeremiah) before going to Nest (Vestige of Loash). He didn't spawn me in Nest after encountering him at Leprosarium. In this case I thought that I had voided this achievement but I still tried to exhaust every encounter of this character when I met him.
After Leprosarium vestige I met him at Abbey (Vestige of Bloody Pilgrim), exhausted he's dialogue and went towards Judge Cleric boss. Before going against Judge Cleric, I checked he's armor location just in case and noticed that there was indeed he's armor on the ground surprisingly.
The actual "route" where he spawns and the kill of the Judge boss;
1. Pilgrim Perch Ascent - On your right just after the first ladder.
2. Forsaken Fen, start - After the Lookout (Vestige of Olleren) on your right before Congregator of Flesh boss.
3. Forsaken Fent, finish - After killing The Huss Saint and cleansing the beacon you will exit the arena and will find Vestige of Pale Butcher shortly after. He is near of this Vestige.
4. Skyrest Bridge - Inside of the main hub area, near the Vestige.
5. Fief of the Chill Curse, near end - Next to Nest (Vestige of Loash) before the Hollow Crow boss fight.
6. Path of Devotion Memorial - Near the Vestige of Dieter, just spin around 90 degrees left and you should find him.
7. Leprosarium - Above of the Vestige of Brother Jeremiah
8. Abbey of the Hallowed Sisters - Near vicinity of the Vestige of Rosamund. This is last location and change to buy him anything before he will disappear.
9. After these steps, kill Judge Cleric and she will drop you "Thorned Chalice" item and pam achievement unlocked.
10. Make sure to get this merchant armor also, it is located above the balcony of the cathedral of the Judge Cleric. Nearest Vestige is Empyrean (Vestige of Iorelo the Cursed Knight). Before going in the cathedral, make a turn left and you will see a little broken tower that has a staircase. Ascend the stairs, go umbral and cross the broken tower. There go though the gates on your right and straight ahead there should be the armor lying on the ground.

Missed locations 5, 6 and 8. Exhausted dialogue everywhere else and got the armor and achievement without issue after beating Judge Cleric.

I also missed a few of the locations, especially due to doing areas out of order. I did speak to him in the last location where he warns you that he's about to leave though.
