To lower threat in a region you need to take over Scrotus camps, clear minefields and destroy convoys/scarecrows/snipers.
Personally I would leave this till after the story, unless you plan on doing the races. I would instead pile your scrap into upgrading Max. Some of the later regions can become quite difficult if your Max is underpowered. It’s also a good idea to upgrade the cars attack abilities like the Thunderpoon to help deal with enemy cars. After the story you’ll have a huge grind if your clearing all regions of threat and you’ll start to get a large intake of scrap from your taken over camps and looting locations, to purchase upgrades.

You will not be able to unlock this trophy until you progress most of the way through the story and reduce threat to zero in the territories controlled by Jeet and Pink Eye. You also require a significant amount of scrap to purchase all required engine upgrades. Once you have purchased the most powerful V6 and V8 engine upgrades available through the Garage menu, the trophy will unlock.

The V6 and V8 engines are the two tiers of engine types in the game. You'll start with a V6 and can spend scrap to upgrade it as they become available. Once you progress enough in the story, you'll steal a V8 engine and have access to that tier of engines too.
In addition to needing scrap to purchase each upgrade, you'll also need to do some other activities such as Wasteland missions and reducing threat in regions. The final V8 engine isn't available until you've eliminated all threat in Pink Eye's territory. Because of that, this will be one of your last achievements. On top of that, you'll need a solid 30,000 scrap to buy all V6 and V8 upgrades to unlock the best ones, so it's best to do this late in the game when you don't have anything more important to spend scrap on.
