To play the game you will have to toss one of 4 fruits to audience members in 4 separate sections of the seating area. The fruits to toss are Bananas (A button), Mangos (B button), Coconut (X button) and Pineapple (I think... Y button).
So if an audience member is asking for a banana (symbol above their head), move left or right to their section of the stands and toss the right fruit to get points. If you are too slow they sit back down and the fruit you toss ends up hitting them and your streak is broken. Same thing applies if you toss a fruit into a section of the stands that nobody is asking for. The more correct fruit you toss, the better your score. I am not sure what the cutoff for 5 stars is as I never experimented with 5-star cutoffs.
In earlier cities there are less fruit options, only banana and mango in the first city, thus the game is easier here. It would be easier to net the achievement in one of these earlier stages although 5 stars is required in all cities to complete the game to 100% so better to get good at it now :-)

This one is similar to the ticket selling circus act as you have to throw fruit to audience members who are requesting it. Each fruit will be assigned to a specific button and you will have to run back and forth along the floor, get in front of the audience member requesting something and push the proper button to throw it to them. Keep moving and throwing the correct fruit to the audience members to raise your score. The easiest place to get this trophy is in ROME as there are only 2 fruits/buttons to deal with there. If you are struggling to get a high enough score you can also put it on 2-players and just have the second player stand there so you can get all the audience members for yourself. After you finish the act and obtained 5 stars in it, the trophy will pop!
