A 2pt conversion is when you have control of the ball after a touchdown. Usually, you can kick the ball for 1 pt, but you can also try to make a conversion, either by doing a pass or a run play, which gives 2 points. You then have 1 attempt to enter the touchdown zone with the ball. If you fail, the opposing team gets the ball (as if you failed a kick).
This achievement asks you to do at least 3 conversions using rushes in one game. This requires you do at least 3 touchdowns, and then three successful conversions using runs.
You can adjust the sliders, modify your team and the opposing team, change the quarters length, etc.

How do you change to a conversion instead of a kick? All 16 choices are field goals :(

Go back into your general plays and pick a run or a pass. There are also plays called Fake FG (and Fake Punt), where your team take a FG formation, but then switch to a pass or run. I don't quite remember this game, but you may have to back out from the first playbook, which automatically assigns a FG attempt, in order to access other play options. Hope this helps.

Once you score a touchdown, instead of choosing to take a field goal select either a "Half Back Dive" or a "Full Back Dive" and make it into the end zone. Repeat this two more times.
