Select a PA (play action) pass play. To abort the play action press the 
(right trigger) after hiking the ball. Any pass play that starts with PA is a play action pass play.
You must score a touchdown to earn the achievement. I suggest being down in the Red zone (20 yard line) or closer to get this.cancels the play action motion, however you still need to pass the ball and score the touchdown to earn this achievement.
I got this doing the Tebowing achievement during the Tebow Madden moment. You cannot do this using Supersim Online Team Play or co-op but I would guess that 2 controller solution (versus) would work for this if you are having trouble.

Got this achievement, but good note on the Tebow cheev too. I know which one I'm doing next!

Choose a Play Action play to run, when you are within 20 yards to the endzone. After you hike the ball, quickly hit 
and throw a touchdown to one of your receivers. If you happen to fail, then try again. Trophy unlocks after a successful try.

Simply call a Play Action pass, and hold RT after you snap it to abort it. Then throw a TD on that pass.
