Pretty simple. Either create a new team or you can get it by using an existing team in Madden Ultimate Team. Achievement will pop after all the introductions and help bits pass. Skip those by pressing 'A'.

Unachievable due to server closure
From the Main Menu select Ultimate Team. Here you will create your own ultimate team. You can choose any team name you want as long as it is not taken already or you can use last year's MUT team name you used. The game gives you a starter pack of cards with most cards having a contract of 25 games.

On the main menu, go to Ultimate Team and follow the steps to create your team.
