Go for a User Pick by hitting the . The achievement unlocked for me immediately after not unlocking for several successful swat attempts.

I did this with 2 controllers and used the swat mechanic ( ). You can follow these steps with 1 controller as long as you're on defense and the offensive team pass the ball.

With your second controller choose:
Formation > Hail Mary > Hail Mary.

With your main controller choose:
Concept > Cover 2 - Any of these seem to work.

1. With your second controller, snap the ball with to begin the play, and wait a few seconds for everyone to run out. Watch for players that are being closely covered by the defenders.

2. Once you pick a receiver to throw to (making sure there's a defender nearby), throw the ball and hit on your main controller to play as the receiver closest to the ball, then hit to swat the ball before it lands.

I did adjust the sliders for 'Pass blocking' under the Player Skill menu in settings. Not sure if this had a big impact but worth doing if you're having trouble.

You may end up getting this one organically just by playing the game, but if you haven't then this one is pretty simple.

When on defense, wait until the QB throws the pass and press B to switch to the defender closest to the ball and press Y to swat the pass away. You can just press Y to swat it, or hold Y to go for the interception. If you intercept the ball, you might not unlock the achievement right away. When I went for this achievement, I intercepted the pass and the achievement did not unlock. On my next defensive series, I accidentally intercepted the pass again, but this time the achievement popped. So keep in mind it may be a little glitchy, but it should unlock after one or two successful attempts
