In the 4th quarter before you select a play click on coaching adjustment then change one of the sliders to aggressive or consevative. Then choose a play, achievement will pop before the ball is snapped.

I have tried over and over but it won’t unlock. Anyone else having issues?

Won't unlock in a two player exhibition match for me.
EDIT: Unlocked when backing out to the main menu.

you can do this on offense of defense, in exhibition or franchise mode
During the 4th quarter, before you select a play back out of the screens with the
button. Scroll down to Coaching adjustments and change one of the sliders from its preprogramed balance to either the left or the right. Back out of the screen and select a play. The achievement should pop, but is known to be buggy.

To unlock the Situational Awareness trophy in Madden NFL 19, wait until the 4th quarter of the game, and then before one of the plays, press 
to back out, scroll down to Coaching Adjustments and switch one of the sliders to aggressive or conservative.
Then choose a play as normal, the trophy will unlock before the ball is snapped.
