This took about 3 tries for me. Pick a team with a good QB (Saints) vs a terrible D (Browns). On offense, pick Deep (B). Defense, pick Short (X). Drop back and heave it. It also really helps if you're lucky enough to get Entourage from the get go.

I had basically given up on this achievement as I played fifteen straight games without getting the ball first, luckily I got the fumbilitis powerup on my first defensive snap and scored a defensive TD so it is doable that way for those like me where the CPU just won't let you start with the ball.

Pick the Cardinals, and play against the Lions...
Make sure you get the ball first, if not exit the game and try again.
Pick Deep Pass Play, and throw it to Larry Fitzgerald (he is number 11 and is on the right side)... Let him run out a bit, about middle of the field and do a lob throw (smash button lightly)...

Fitzgerald is the MAN!!! if it helps anyone i found out that having the fast past (bullet) active was a great help and your more likely to do it if u wait for fitzgerald to move into the middle of the field on his run.

To start, this achievement cannot be done if you don't start out with the ball. Even if you score on your first play with the second drive you will not get this achievement.
You can do this achievement online if you will, just do a deep pass to your left or right side receivers and have the opponent pull them away from their man creating an opening for an easy TD.
Legit: To do this against the CPU I used the Eagles against the Lions. Desean Jackson is one of the fastest players in the game, so have him go deep and throw a lob about mid field or farther. Chances are he will burn his man and as long as you sprint and if you play against the Lions the pass defense will be weaker than most making this achievement that much easier.
EDIT: MattiasAnderson confirmed that if you score a defensive touchdown on the first play, it does in fact count. This might be a bit tougher to do however it is still possible.

What about if you get a safety, will that count on the first play?

I haven't tried that, however I'm doubting it because the game expects a touchdown you could give it a try however. You could most likely do two controllers as well if that's any easier.

Start a game using "Play Now" and leave the settings as is. As the description states, score on the first play of the game. The achievement will unlock once you meet the requirements. You do not have to complete a full game.
Two Controller Method:
As controller one (main profile,) run a long pass play and select a short pass defense on controller two. Hike the ball and switch to the defender and move him away from the player you're throwing the ball to. Run in for the touchdown.
