the tunnels section seems pretty confusing to navigate, but as the trophy called "follow me" it is exactly what you need to do to find the exit, once you navigate the tunnels you will see a madison demon figure wander around the section (not including the jumpscares), just follow to the path she is going and you should see the exit where there are graves at its end.
be advised that at some sections madison will not always show the way or your light might go out and you will miss her so to make it easier just use the video guide below.
but I can't tell if this is randomized, as some stuff in the game are different for every player, so keep in mind that if you see a different path from the video just keep track of madison until she leads you out
