After completing the final mission of the expansion, "...Worse Than Dying," Father James will inform you that Aunt Lily is coming to town. At this point, you will receive a new objective on your map to pick up Aunt Lily from the local bus stop.

From there, pick up your relative, drive her to Sammy's Bar, and then speak to her about renovating the building. The first renovation option available is redoing the first floor and basement. This will cost $20,000. Upon making this first of many renovations to Sammy's, the achievement will unlock.
