Getting 5 balls on the screen will require you to active two multiball powerups. Each multiball power up creates two new balls.
This can be done on stage 6-2. Restart the level until the two multiball powerups are conveniently placed near the start and destroy them as soon as possible. 3 seconds is a very short period of time, so just concentrate on any balls that look they may immediately be heading out of the playing field.
