A double play is two outs in one play. Simple enough. While pitching, an opposing runner has to be on base. It's easiest when their on first base.
The key to this is getting a ground ball in play. Fly balls usually reach the outfield, and a line drive is too quick to get a second out.
While pitching, keep the ball low in the strike zone. Once an infield grounder is hit, throw it to second base, then immediately throw it to first base. Once the second out is recorded, you'll get the achievement.
Not obtainable in My Player mode.

There are numerous opportunities for a Double Play to happen in game. The most likely occurrence is when a batter hits a ground ball to the Second baseman or shortstop and the first out is made at second base, (you must have a runner on first base), and then the Second baseman throws the ball to first base to get the batter out.
