Did this accidentally by overthrowing from third to first. And apparently with two outs. Should be easy enough to do. Just hold down the button till the meter is way in the red. You will most likely pull the first baseman off the bag.

With 2 outs in an inning, you need to allow a hitter get a hit off you. The hit needs to be a single. When you are throwing to first base, hold 
till the markers is in the red, and once in the red release . Being on red means your throw will be off, and thus you will miss your throw to first.
