How to unlock the To the Rescue achievement in Major League Baseball 2K11 - Definitive Guide
For this achievement, you have to have a one (1) to three (3) run lead on your opponent when you send the last pitcher in just to get a save, but specifically for this achievement you can only have a max of 3 because technically you could send a reliever in for the 8th inning with a 2 run lead, score like 6 runs and still get credit for a save if you keep the same guy in since he came in during save requirements...
My advice is to be up by just 1 going into the 9th. Get two people out, then intentionally walk someone before you bring in your closer, be sure he's warmed up before you bring him in. Once this is done, get the final out and the achievement will pop up afterwards. You can do this either against the AI, or in a ranked or league match online.

Any idea why this is marked as Buggy? I have been playing mostly "My Player" mode so I haven't even got into a franchise yet to start to get this.

This won't unlock playing "My Player Mode" and last I heard, this isn't one of the buggy achievements at all..

The achievement description isn't exactly accurate. A save in this game and a save in real baseball apparently aren't the same. For this achievement you need to come in and get the save when a runner is ALREADY on base. So you can't put the tying run on with the guy you're trying to get a save with.
Quickest way for this is to start up a quick game. Pause the game and turn down the opposition's hitting sliders. Keep throwing fastballs right down the middle. They will almost always be strikeouts or groundballs. When you are at bat bunt every time. When you get it down keep hitting LT to advance to the other bases. This will get you out quick.
Now all you need is 1 run. So pause the game turn down the oppositions pitching and fielding sliders and turn up all your hitting sliders when you are going for this. After you get the run turn the sliders back to normal. Now in the 9th get 2 outs and hit a batter. Now bring in a new pitcher. Get the out and the achievement will pop.
If you get to the 9th and you are leading by more than 3 runs just keep hitting batters until the score is 3 or less and enough runners are on base to tie.

In the ninth inning, you must be up by three runs or less, and load the bases with enough batters to have the tying run on base. Its always best to have your closer warming up in the 8th inning and then bring him in the 9th. You cannot allow the tying run to score.
